Physical Therapist | Cottonwood, California

An exciting opportunity is available for an experienced Physical Therapist to provide exceptional care in a Home Health setting at a hospital in California. This contract position focuses on serving a diverse patient population, primarily those recovering from orthopedic procedures or geriatric patients with complex comorbidities such as CHF and COPD.


Schedule:

• Day shifts
• Occasional Saturday rotation for orthopedic patients
• Holidays off

Key Qualifications:

• Current CA Physical Therapist (PT) license required
• Active BLS certification
• Minimum of 2 years of recent physical therapy experience, with hospital or home health background preferred
• Familiarity with EMR systems (Netsmart a plus)
• Skilled in conducting Start of Care (SOC) visits, particularly with orthopedic patients when nursing services are not ordered
• Comfortable working independently within a multidisciplinary team

Primary Responsibilities:

• Conduct an average of 5 home health visits per day throughout most of Tehama County, with efforts to minimize driving distances
• Provide high-quality rehabilitative services to orthopedic and geriatric patients
• Accurately document evaluations and interventions using a provided laptop and EMR software
• Collaborate with intake RN, manager, and other therapy staff to ensure optimal patient care
• Participate in flexible scheduling, including rotating occasional Saturday coverage
