We're partnering with a nationally recognized inpatient rehabilitation hospital seeking an experienced RN Case Manager to join its interdisciplinary team in Woburn, MA . In this role, you'll collaborate with physicians, therapists, nurses, patients, and families to coordinate individualized care plans, facilitate safe discharge planning, and ensure exceptional patient outcomes throughout the rehabilitation process. Responsibilities Coordinate patient care from admission through discharge. Develop individualized discharge plans with the interdisciplinary care team. Collaborate with physicians, nursing, therapy, patients, families, and community resources. Educate patients and families throughout the rehabilitation process. Promote quality outcomes while ensuring regulatory compliance. Qualifications Active Registered Nurse (RN) license. Minimum Associate Degree in Nursing (BSN preferred).
