Mental Health Wellness CounselorLocations: New York, NY Hourly Rate: $23.00 Available Shifts: 9am - 5pmSchedule(s):Monday - FridayTuesday - SaturdayDepartments:Homeless ServicesMental HealthOverview: Works independently in the field, as part of a rehabilitation services team, with people of diverse ethnicities, backgrounds who have a mental illness and are living in the community, in single and scatter-site apartment and SRO housing.Requirements:Bachelors degree2 years of case management experience in a social service agencyResponsibilities:Engage people with mental illness and assist them in meeting the demands of community livingAssist in maintaining their housing by teaching them the necessary skills, and linking them to services that will support their recovery, independence and self-determinationConduct Group Meetings on Wellness ManagementComplete service plan reviews for all residentsRegularly assess, correct and report apartment and personal living space inspections conditions as assignedProvide Case Management for residents, engage residents in setting rehabilitation and housing support goals, and collaborate with them in developing a Housing Support Plan
