Registered Nurse (RN) - Case Manager - Full Time

Position Overview Building Location: 32nd Avenue Building, Department: 3015320 – CASE MGMT-SOCIAL SERVICES, 32ND HOSP. Job Responsibilities Responsible for organizing and providing care to patients through assessment, planning, intervention, and evaluation. Supports the physician and interdisciplinary team to assure patient progress through the continuum of care to discharge, enhancing the quality of clinical outcomes and patient satisfaction while managing the cost of care and providing timely and accurate information to payers. The role integrates and coordinates utilization management, care facilitation, and discharge planning functions, and is accountable for planning effectively to meet patient needs, manage length of stay, and promote efficient utilization of resources for a designated caseload. Qualifications BSN or ADN degree from an accredited school or college of nursing. Current RN licensure in appropriate state(s) of employment. Preferred experience in case management or acute care nursing (2–4 years of experience). Basic Cardiac Life Support (BCLS) not required. Schedule and Work Hours Salaried position Monday‑Friday 8:00AM – 4:30PM, with limited weekends and holiday rotation. Possible remote/hybrid option. Shift rotation: Day shift, 8:00AM – 4:30PM. Weeks and holidays limited.
