Medical Social Worker (MSW)

Location: Mission Viejo, CA
Pay Rate: $35 - $40/hour
Duration: 13 Weeks
Shift: Day Shift
Schedule: 5x8 Hours | 8:00 AM – 4:30 PM
Weekend Requirement: Every Other Weekend
Floating: Required to Laguna Facility

Position Overview

We are seeking an experienced Medical Social Worker (MSW) for an acute care hospital setting. The MSW will provide psychosocial assessments, discharge planning, crisis intervention, and supportive counseling services to patients and families. This role requires strong clinical judgment, effective communication skills, and the ability to manage a diverse patient caseload in a fast-paced healthcare environment.

Responsibilities

• Conduct comprehensive psychosocial assessments

• Develop and implement discharge plans

• Coordinate care with physicians, nurses, case managers, and community agencies

• Provide crisis intervention and emotional support to patients and families

• Facilitate referrals to community resources and support services

• Address barriers to discharge and continuity of care

• Advocate for patients throughout hospitalization and transition of care

• Collaborate with interdisciplinary healthcare teams to optimize patient outcomes

• Maintain accurate and timely documentation in accordance with facility and regulatory standards

• Manage a caseload of approximately 20–23 patients

Requirements

• Master's Degree in Social Work (MSW) from an Accredited Program

• Minimum 1 Year of Recent Acute Care Hospital Social Work Experience

• Previous Travel Experience Required (First-Time Travelers Not Accepted)

• Strong Experience with Discharge Planning, Care Coordination, and Psychosocial Assessments

• Knowledge of Community Resources, Insurance Programs, and Post-Acute Care Services

• Excellent Communication, Documentation, and Interdisciplinary Collaboration Skills

• Ability to Manage a High-Volume Caseload

• Experience with Electronic Medical Record (EMR) Systems Preferred

Preferred

• California Social Work License

• Pending California License Accepted (Must Be Active by Start Date)
