Certified Nurse Midwife Advance Practice Provider (APP)

???? Full-Time or Part-Time | Outpatient | 40 Hours/Week

Seeking a bilingual OB/GYN provider to deliver prenatal, obstetric, well-woman, and gynecological care in an outpatient setting. The provider will focus on preventive care, patient education, and promoting healthy lifestyles while working within a collaborative care team.

• Provide prenatal, obstetric, gynecological, and well-woman care

• Conduct patient assessments, physical exams, diagnoses, and treatment plans

• Prescribe medications and coordinate referrals as needed

• Maintain productivity standards (approx. 2.3-2.6 patients per hour)

• Provide patient education on preventive care and healthy lifestyles

• Participate in quality improvement and community health initiatives

• Maintain accurate clinical documentation and continuity of care

• Graduate of an accredited Nursing program with advanced training

• Active Illinois license , DEA, and Controlled Substance registration

• Bilingual English/Spanish required

• Strong communication and patient care skills

• 40 hours per week (4 clinic days + 1 admin day)

• 1 Saturday per month
