Whether in the comfort of home or in a facility, our mission is to enhance the quality of life for every patient we serve.We are growing rapidly and seeking a skilled, compassionate, and motivated **Physical Therapist (PT)** to join our team in**.** If you're looking to advance your career in a supportive environment that values excellence, autonomy, and teamwork, we’d love to meet you.**Position:** Physical Therapist (PT)**Pay Range:** $65-80/hr**Status:** Part Time **Coverage Area:** Beaumont/Banning/Redlands/Moreno Valley**What You’ll Do**The **Physical Therapist (PT)** is responsible for delivering high‐quality physical therapy services in accordance with the physician’s plan of care and all requirements outlined in the Personnel Services Agreement. The PT plays a key role in patient recovery, functional improvement, and interdisciplinary care coordination.* Provide skilled physical therapy services based on the written physician’s plan of care* Conduct comprehensive, ongoing assessments and collaborate with the patient, caregiver, and interdisciplinary team to update and individualize the plan of care* Educate and counsel patients, caregivers, and families on therapy goals, safety, and home exercises* Prepare accurate and timely clinical documentation, including visit notes and progress summaries* Communicate regularly with the physician and healthcare team regarding patient progress and changes in condition* Initiate and implement the physical therapy program, instructing caregivers and appropriate staff on techniques they can safely perform* Participate in case conferences to review goals, progress, and care plans* Train patients in the proper use of prosthetics and assistive devices* Identify needs for additional home health services and coordinate referrals when appropriate* Participate in patient discharge planning and recommendations for continued care* Provide in‐service education and training to agency staff as needed* Engage in peer consultation and quality improvement processes* Supervise Physical Therapist Assistants (PTAs) in accordance with state and organizational guidelines**Requirements*** Active CA Physical Therapy License* Strong verbal and written communication skills* Strong organization skills* Possesses and maintains current CPR certification* Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order**Why Join Bella Terra Home Health & Hospice?**Bella Terra is part of the Cornerstone Group, a nationwide network of 75+ home health and hospice agencies.
