Humana seeks a Field Care Manager, Behavioral Health 2 to assess and manage members’ behavioral health needs and guide them to resources for optimal wellness. You will coordinate care across providers, support transitions, and conduct field visits to ensure positive health outcomes. Travel up to 75% (30–40 miles) with quarterly meetings in Louisville, KY. A Master’s in Behavioral Health and active Kentucky licensure or equivalent RN with behavioral health experience are required, along with 2+ #J-18808-Ljbffr
