Job Details Location: Canyon Colorado Home Care & Hospice — Loveland, CO 80538 Position type: Full Time Salary range: $90,000.00 - $105,000.00 per year Overview We are looking for a compassionate and enthusiastic Clinical Manager to join our Home Health team. Our team embraces an upbeat attitude and a constant drive to learn and grow. We work together to provide the best possible service and pride ourselves on quality patient care. The Clinical Manager supervises the activities of the clinical staff, plans and evaluates professional health services with the agency, and confers with other administrative staff to assure that services are provided at the highest possible quality level and are consistent with professional standards and goals. The Clinical Manager is qualified to act in the absence of the Administrator. Previous Home Care and/or Hospice experience is preferred, but not necessary. Reliable transportation is required. We offer flexible scheduling and a generous Paid Time Off plan for our full-time employees. We also offer highly competitive compensation rates plus health, dental and vision insurance. Compensation will be based on level of experience. Typical Responsibilities Plan, direct, and participate in the delivery of professional services Establish and implement priority timelines for services Develop, recommend, implement, evaluate and interpret policies and procedures relevant to the provision of health services and monitor compliance of the same May be required to work after normal business hours to accommodate client needs and new referrals May require on-call rotation to cover weekend services Ensure agency growth by providing excellent patient service, strengthening clinical teams and supporting health care needs in the local community Oversight Making patient and personnel assignments Coordinating patient care Coordinating referrals Assuring that patient needs are continually assessed Assuring the development, implementation, and updates of the individualized plan of care, which would entail communication with all physicians involved in the plan of care and integration of orders from all physicians involved in the plan of care, including those orders related to medications Responsibilities Understand and adhere to all applicable regulations, standards, and federal, state and local laws and established agency policies and procedures Participate in the development of standards which ensure safe and therapeutically effective service to patient/family Supervise, guide and develop patient care personnel skills and performance Consult with the Administrator in determining a staffing pattern which will accomplish stated objectives and promote a maximum level of utilization of patient care personnel Assist the Administrator with recruiting, hiring, evaluating and terminating personnel. Assist in instituting, interpreting and documenting disciplinary action Responsible for the orientation of new patient care personnel Assist in the planning and consultative needs of staff Qualifications Must be a licensed physician, physical therapist, speech-language pathologist, occupational therapist, audiologist, social worker, or a registered nurse Acceptance of philosophy and goals of the Agency Ability to exercise initiative and independent judgment Ability to work with individuals to enlist cooperation of many people to perform or achieve a common goal #J-18808-Ljbffr
