Care Coordinator.

Care Coordinator Job Description Care Coordinators are part of the Addiction Response Team and work collaboratively with local providers and community agencies to ensure individuals with Substance Use Disorders are connected with resources that support their long‑term health, treatment, and recovery goals. They aid individuals in meeting their recovery goals through education, outreach, and connectivity to medication‑assisted treatment (MAT) and other services. They maintain frequent communication with partner organizations to provide updates, identify barriers, and avoid duplication of services. Knowledge, Skills, and Abilities Experience working with and building trusted relationships with persons with Substance Use Disorder, especially opioid, alcohol, and stimulant use disorder. Cultural humility and ability to engage with diverse populations including People of Color, LGBTQIA+, persons with disabilities, veterans, low/no income, homelessness, criminal justice involvement, mental health conditions, and complex medical conditions. Critical thinking and problem‑solving skills. Customer service skills. Strong decision‑making skills. Strong verbal and written communication skills in person, phone, and email. Ability to remain non‑reactive and calm when engaging with individuals who may be intoxicated, belligerent, or have a history of behavioral health issues. Ability to adjust priorities quickly as circumstances and needs dictate. Computer skills including navigation of multiple electronic medical record systems and data collection tools as required. Experience partnering with professionals in local and regional health. Experience working with Medicaid and Medicare. Bilingual preferred. Ability to work independently in a busy office environment with interruptions. Reliable transportation. Education or Formal Training HS Graduate or GED required. Licensure / Certifications Valid Colorado Driver License and insurance required. Working Environment / Physical Activities Frequent contact with the public by phone and in person. Typical duties include sitting, standing, walking, climbing stairs, bending, reaching, and lifting up to 30 pounds. Must be fully vaccinated against COVID‑19 and receive annual influenza vaccine. Job Responsibilities Enroll persons using appropriate assessment tools and complete GPRA survey at enrollment and regular 6‑month follow‑ups. Support enrolled persons in continuing engagement with medical and behavioral health treatment plans; assist with scheduling appointments, referrals, medication refills, etc. Provide health needs assessments, identify risk factors, and develop health plans that integrate treatment and recovery services, including MAT. Monitor members according to NCHA policy and addiction response team guidelines. Conduct telephone contacts, office/hospital visits, and other contact methods to reach identified goals. Attend member appointments with providers and community resources as needed. Participate in health promotion and education activities as identified in member care plans. Collaborate, communicate, and coordinate among multidisciplinary team members to support goal achievement and reduce duplicate services. Optimize member and family independence through education, community resources, and support. Review cost‑effective care options and strive to reduce duplication of services. Communicate and facilitate patient complaints, grievances, and appeal processes. Participate in regular case review with supervisors and trainers. Maintain strict confidentiality of member records and communications. Complete Certified Addiction Technician classes and attend supervision as required. Safely transport members to and from MAT appointments in NCHA company vehicle. Other Duties as Assigned Assist with tasks designated by NCHA leadership to support overall goals. Provide input into development of policies and procedures. Support the mission and vision of NCHA. Comply with NCHA procedures and with policies of partner agencies. Attend internal and external meetings and complete in‑person and virtual training as identified. Participate in ongoing performance improvement activities.
