Care Manager I, Homeless Services (Nashville, TN)

Mental Health Cooperative (MHC), founded in 1993, serves individuals with severe and persistent mental illness and, since 1999, children and adolescents with severe emotional disorders across Middle and East Tennessee. Job Summary Care Manager I – Homeless Services (Nashville, TN). Salary: $43,000 base (plus incentive for language skills and behavioral health experience). Sign‑on bonus: $1,500. Schedule: Mon‑Fri, 8:00am‑4:30pm. Mileage reimbursement available. Responsibilities Provide care management services to homeless or at‑risk consumers with severe and persistent mental illness. Conduct continual assessment of consumers’ needs and develop individualized service plans and crisis plans with measurable goals. Assist with referrals and linkage to community resources, advocate for consumers whenever necessary. Ensure consumers are seen upon discharge from the hospital and within Agency standards. Plan and coordinate daily psychiatric, medical, and environmental needs. Facilitate efficient transition of consumers to community settings and coordinate with Agency and community partners. Meet productivity standards per Policies and Procedures; meet regularly with the Care Management Supervisor. Attend and facilitate morning and weekly team meetings; follow up on plans developed in each meeting. Facilitate education of consumers and involved collaterals about psychiatric diagnoses and treatment options. Respond to urgent/crisis consumer needs with supervisor, crisis services, and clinic staff (developing plans and interventions). Develop and maintain relationships with family and friends; create community relations through prompt follow‑through. Ensure timely and appropriate documentation as required by the Agency. Participate in all required PATH trainings and community coordinated events, campaigns, and initiatives as directed by Program Manager. Requirements Bachelor’s Degree in counseling, psychology, social work, or another behavioral health field. Candidates with a bachelor’s degree in criminal justice must have fifteen college‑level semester hours of coursework in behavioral health. Valid Tennessee Driver’s License. Acceptable Motor Vehicle Report (MVR) and Criminal Background Investigation. Personal Automobile Insurance. About You Team oriented; attends and participates in team meetings. Can assist other care managers as needed. Shows recovery focus and develops individualized service plans. Documents progress notes and service plans in a timely and high‑quality manner. Demonstrates flexibility, time management, prioritization, and self‑discipline. Exhibits empathy, strong listener, and crisis management skills. Shows integrity, trust, honesty, professionalism, and dependability. Effectively engages with consumers, families, and agency staff. Competencies Effective communication with adults, families, and collateral contacts. Uses best‑practice standards and documentation to link goals to care plans. Completes accurate assessments (PRAPARE, DLA, CANS, Columbia Suicide Risk and Severity). Advocates for consumer needs to improve daily functioning.
