Hybrid RN Case Manager - Home Health & Care Coordination

FreedomCare is seeking a Registered Nurse (RN) for our Georgia team in a hybrid role based out of Douglasville. You will perform home visits to assess needs, develop care plans, and ensure ongoing compliance with caregiver and patient goals. The ideal candidate holds a GA RN license, has 2+ years of clinical experience (1 year with elders), and is capable of coordinating with physicians, caregivers, and the program team.
