Overview:

The School Transition Coordinator is responsible for planning, managing, and supporting the smooth transition of students between educational levels or programs. This role involves collaborating with teachers, administrators, families, and community partners to create a positive, structured, and supportive environment that helps students adjust to new academic and social settings.


Responsibilities

Responsibilities:

• Develop and Implement Transition Plans :
  • Collaborate with students, families, special/general education teachers, and IEP teams to create individualized transition plans based on the student's strengths, preferences, interests, and needs.
  • Conduct or coordinate transition assessments (career interest inventories, vocational evaluations, self-determination assessments, etc.).
  • Write measurable post-secondary goals (education/training, employment, independent living) and aligned annual IEP goals/transition activities.
• IEP and Meeting Participation :
  • Attend and contribute to IEP/ARD (Admission, Review, Dismissal) meetings, especially for transition-related discussions.
  • Serve as administrator or liaison in meetings and ensure compliance with transition requirements.
• Coordinate Services and Supports :
  • Link students to community agencies, vocational rehabilitation services, adult service providers, employers, and post-secondary programs.
  • Facilitate interagency collaboration and serve as the district liaison to outside agencies.
  • Develop and coordinate work-based learning experiences, internships, job placements, job coaching, and community-based instruction.
• Student and Family Support :
  • Help students develop self-advocacy, self-determination, and independent living skills.
  • Provide information, training, and resources to families on transition topics, benefits, and adult services.
  • Support career exploration and preparation for employment (e.g., work habits, interview skills, transportation).
• Program Development and Compliance :
  • Oversee or support adult transition programs, career preparation, and employability services.
  • Ensure compliance with state performance plan indicators and federal/state regulations.
  • Collect and analyze data for reporting and program improvement.
  • Develop policies, procedures, and training for staff on transition best practices.
• Additional Duties :
  • Build and maintain partnerships with employers, community organizations, and agencies.
  • Monitor student progress in transition activities and adjust plans as needed.
  • Provide professional development to school staff on transition topics.
  • Administer or coordinate assessments and maintain records/documentation.

Qualifications

Qualifications

• Bachelor's degree in Education, Counseling, Social Work, or a related field (Master's degree preferred).

• Demonstrated experience in educational administration, counseling, or student support services.

• Strong organizational, communication, and interpersonal skills.

• Ability to work effectively with diverse groups, including students, families, and school personnel.

• Familiarity with student support services, special education practices, or related areas is an asset.

Posted Salary Range

USD $20.00 - USD $28.00 /Hr.
Physical Requirements

Physical Requirements

• Ability to run short distances and move swiftly in response to student needs, including bending, kneeling, and crouching.

• Ability to lift and assist students with mobility challenges, often requiring the handling of up to 50 pounds or more.

• Skill in managing fine motor tasks, such as helping students with writing, manipulating small objects, or using adaptive devices.

• Ability to operate a computer or tablet for up to 8 hours daily.

• Capacity to notice and respond to non-verbal cues from students

• Capacity to remain calm and composed during physically and emotional demanding situations, ensuring student safety and well-being
