Hospice Registered Nurse (RN) - Admissions & Care Transitions Specialist

Job Title: Hospice Admissions RN – Be the First Voice Families Trust (Registered Nurse)

Company: Superior Hospice, part of the Home Health Care, Inc. family

Compensation: $200 – $250 per admission

Benefits: Full Benefits Package for Full-Time employees

Location: Greater Twin Cities Metro Area, 21-county service area (territories assigned based on coverage needs)

Schedule: Full-Time or Part-Time with 1 on-call shift per week (backup) and every 4th–7th weekend

What You’ll Do

• Travel to patient homes, assisted living, and care facilities to complete hospice admissions.

• Perform clinical eligibility assessments and initiate services .

• Lead goals-of-care conversations with patients and families.

• Provide clear education on hospice philosophy, services, and expectations .

• Coordinate with intake, physicians, and the interdisciplinary team.

• Ensure a smooth, confident transition into hospice care .

• Maintain accurate and timely documentation .

What You Bring

• Active Minnesota Registered Nurse (RN) license.

• Experience in hospice, oncology, palliative care, or high-acuity hospital settings .

• Strong ability to communicate clearly and build trust quickly .

• Confident clinical judgment and decision-making skills.

• Comfort working independently in a field-based role .

• Reliable transportation and willingness to travel.
