Part-Time Outpatient RN Case Manager | Care Coordination

Harbor Healthcare System in Fort Worth is seeking a part-time RN Case Manager for an outpatient setting to coordinate admissions, assessments and discharges as part of the interdisciplinary team. Requirements include a Texas RN license and 1+ year RN experience ( Hospice/Home Health preferred), excellent communication, ability to travel with reliable transportation, and a clean background with drug screen and driving record.
