Family Practice Physician La Red Health Center Inc – Georgetown, Delaware, United States About this Position Job Summary: Provides medical services within the scope of licensing, training and credentialing to individuals throughout their lifespan. Provides continuing, comprehensive health maintenance and medical care to entire family regardless of gender, age or type of medical problem. Essential Responsibilities Coordinates a program of comprehensive health for patient and family unit which includes preventative medicine, behavioral sciences, and community health. Examines patients, formulates differential diagnostic plans, defines and orders diagnostic testing. Interprets examination findings and diagnostic test results and implements treatment plans. Analyzes and interprets data to establish plans for the management of complex health problems. Determines need for consultation and assists in medical care and treatment provided at the direction of other specialists. Manages family practice services for the health care of families. Formulates plans and recommendations for outpatient services directed toward health maintenance and ongoing health care of families by a physician who serves as their personal physician. Demonstrates the ability to assess, plan, implement and evaluate individual patient care appropriate to the age of patients served by demonstrative knowledge of the principles of growth and development over the life span. Perform health maintenance for well patients, including administering or supervising immunizations. Consults with CMO and other professionals on staff as needed regarding patient care, assessment and education issues. Participates in multidisciplinary teams to improve patient care processes and outcomes. Reviews quality assurance data and monitors for acute and chronic care management. Educates patients and families about health status, health maintenance, and management of acute and chronic conditions. Encourages focus on wellness, prevention and early detection by providing materials and resources to patients and their families. Practices medicine in accordance with the current standards of care for Primary Care and in accordance with the criteria adopted by La Red Health Center. Communicates appropriately with the person served regardless of his/her age. Maintains cultural competency and demonstrates an ability to work with diverse ethnic and racial populations. Provides leadership and consultation to clinical personnel. Completes patient EMR documentation in a timely manner. Documents patient assessment and intervention data. Demonstrates skill and proficiency in an electronic medical records environment which includes patient data collection, e-prescribing, patient portals and other electronic means of patient care. Education and Experience Graduate of an accredited medical school with a degree of Doctor of Medicine (MD) or Doctor of Osteopathy (DO) and completion of an accredited residency program. Board certification in specialty within three (3) years, if newly graduated from residency. Unrestricted license to practice medicine within the State of Delaware. Current DEA certificate without restriction. Current Delaware Controlled Substance Certificate. Flexibility to work irregular hours and to work with multi-cultural, diverse patient population. Language Skills Bilingual (Spanish/English) preferred. Ability to converse in French Creole a plus. Knowledge, Skills and Abilities Ability to treat, counsel and educate patients and their families. Strong desire and ability to serve people. Pleasant, professional bedside manner and approach to patients and their families. Ability to respond quickly to emergencies occurring in the Health Center. Maintain an effective working relationship with all LRHC staff. Promote a participatory health environment that includes health care team members, patients and families in the care and health planning for the patient. Ability to work with ethnically diverse cultures. Ability to quickly familiarize and adapt to EMR system. Ability to manage time. Excellent active listening skills. Equipment Operated Wide range of standard medical equipment including portable Ultrasound equipment, nebulizer, syringes, speculum, oxygen tanks, wheelchairs, thermometers, etc. Computer use and proficiency required. Work Environment Medical exam/treatment rooms and medical offices in Health Center. Controlled lighting, air quality, and temperature settings. Environmental hazards may be unpredictable including exposure to communicable diseases and biohazards. Mental/Physical Requirements Involves standing, walking, kneeling, bending, grasping, manipulating and squatting and lifting (up to 50 lbs.). Walking or standing may be more than four (4) hours per day. Must be able to work in a standard medical office setting including use of a computer, and standard office equipment. Must have ability to read computer screens, printed materials and hearing and speech to communicate in person as well as over the phone. Must be able to attend meetings or make presentations at various sites when required. Stress can be triggered by an individual’s response to workload or specific cases. #J-18808-Ljbffr
