Key Responsibilities

• Perform comprehensive Physical Therapy evaluations for a diverse range of orthopedic conditions, ensuring a thorough understanding of each patient's unique needs.
• Develop individualized treatment plans in collaboration with the patient and their family, setting clear, achievable goals for recovery and rehabilitation.
• Implement and modify treatment techniques, applying evidence-based practices to address specific impairments and facilitate functional improvements.
• Utilize clinical assessments to routinely reevaluate patient progress, adjusting treatment strategies as necessary based on performance and feedback.
• Maintain accurate documentation in the Company's electronic record system, including SOAP notes, progress summaries, and treatment plans, adhering to all compliance and regulatory standards.
• Educate patients and their families on the recovery journey, providing insights on the rehabilitation process and addressing their questions and concerns.
• Engage with interdisciplinary teams, collaborating with colleagues across various therapy disciplines to provide holistic care for patients receiving multiple services.
• Promote the clinic's core values and culture, acting as an ambassador for Evolution Physical Therapy Clinic and contributing to a positive workplace atmosphere.

Qualifications
