As a HomeWell caregiver, you will provide personalized, one-on-one care based on each client's individualized care plan. You'll have the opportunity to build meaningful relationships while helping clients remain comfortable, confident, and independent in their own homes.

What You'll Do

Depending on the client's needs and care plan, your responsibilities may include:

• Providing companionship and emotional support

• Preparing meals and assisting with meals

• Light housekeeping and laundry

• Shopping and errands

• Transportation to appointments and other outings

• Medication reminders

• Personal care, including bathing, grooming, dressing, and toileting

• Assistance with walking, mobility, and transfers

• Supporting clients with activities of daily living

• Observing and reporting changes in a client's condition

• Following all individualized care plans and company policies

What We're Looking For

We are seeking caregivers who are:

• Caring, compassionate, and patient

• Mature, professional, and respectful

• Reliable and punctual

• Dependable and committed to their clients

• Comfortable communicating with clients, families, and management

• Able to work independently while following care plans and instructions

• Physically able to safely perform required caregiver duties

Minimum Qualifications

• 1,000 hours of experience caring for seniors or individuals with chronic illnesses preferred
  (approximately 6 months full-time or 12 months part-time)

• CNA or HHA certification preferred, but not required

• Exceptions may be considered for newly licensed CNAs/HHAs and caregivers providing companionship or housekeeping services only

• Current TB test

• Reliable transportation

• Valid driver's license and current auto insurance if transporting clients

• Ability to safely assist with client transfers and other physical care needs

• Strong communication skills

• Ability to pass a criminal background check

• Verifiable professional references

Essential Responsibilities

Follow the Care Plan:
Provide services according to each client's individualized care plan and communicate with the office when needs change.

Provide Quality Care:
Deliver compassionate, respectful, and dependable care while promoting each client's dignity and independence.

Communicate:
Promptly report changes in a client's condition, concerns, incidents, or emergencies to the Care Manager.

Follow Safety Procedures:
Follow infection control practices, safety procedures, and PPE requirements when applicable.

Accurate Timekeeping:
Accurately clock in and out of every shift and document completed tasks using ClearCare Telephony .

Professionalism:
Maintain a professional appearance, follow the company dress code, and represent HomeWell positively in every client interaction.

Reliability:
Arrive on time, communicate appropriately with the office, attend required training and meetings, and follow company policies and procedures.

Who You'll Work With

Caregivers report directly to the Care Manager , with scheduling and shift coordination handled by the Staffing Coordinator .

Our office team is here to support you, answer questions, and help you be successful in your client assignments.

Physical & Environmental Requirements

This position requires the ability to:

• Safely assist clients with transfers and mobility

• Perform essential caregiving duties in a client's home

• Travel to client homes using reliable transportation

• Work in a variety of home environments
