Psychiatric Registered Nurse

Psychiatric Registered Nurse Location: New York, NY 10019 Position Type: Temporary Available Hours 15-20 hours per week Responsibilities Conduct medical health assessments, behavioral health pre and post evaluations, care stabilization, and referral services to clients in need of follow up appointments Work with the Residential Coordinator to update and maintain medication advocacy; ensuring all medication changes are documented and followed. Using a client-centered approach to client care, the Psychiatric Nurse Practitioner will ensure that the clients receive an integrated, multidisciplinary assessment and immediate stabilization plan. Assessments are to include client choice and/or mandated care, harm reduction, trauma-informed care, and psychosocial rehabilitation. The Residential Psychiatric Nurse will perform with a demonstrated ability and sensitivity providing services to highly vulnerable individuals who have history of homelessness, substance affected, and/or have serious and persistent mental illness. Refer individuals to immediate/urgent care as well as establish community connections with primary care, mental health services and substance use disorder care. Provide preliminary health diagnosis, care, and treatment plans to clients. Provide crisis intervention assistance when needed. Document each client interaction, health data and notes in the AWARDS system. Coordinate and follow-up on internal and external doctors' appointments, hospital and rehabilitation admissions of clients including healthcare assessments as needed. Collaborate with the Residential Manager, Buildings and IT Manager and other interdisciplinary staff to prioritize placement meetings for company. Establish and maintain community health resources with direct linkages to care. Participate in internal and external health care management meetings with Senior Director, Program Director, Case Managers, and other care coordinators when needed. Administer medication as prescribed when needed, including but not limited to ointments, injections, ophthalmic, etc. Manage and report quarterly on any an assessment and treatment outcomes.
