Work Environment

Your role will primarily take place onsite in patients' homes, allowing you to foster meaningful relationships with those you serve while delivering therapy tailored to their individual needs. Our modern home care model allows for an adaptable work schedule, where you will have the flexibility to prioritize patient schedules while maintaining a Monday to Friday work week from 2 PM to 8 PM. You will collaborate with a passionate team of healthcare providers dedicated to comprehensive, patient-centered care.

Key Responsibilities

• Deliver high-quality, patient-centered care by managing the complete therapeutic process for an assigned group of patients, ensuring a seamless partnership with physicians and the patient care team.
• Conduct thorough patient assessments to identify needs, setting individualized care plans that emphasize goal achievement, engagement, and positive patient experiences.
• Implement therapeutic techniques and interventions directly in patients' homes, utilizing evidence-based practices that align with each patient's plan of care established by their physician.
• Evaluate therapeutic, rehabilitative, and functional statuses, actively participating in the development of a comprehensive plan of care that reflects the unique needs of each patient.
• Instruct patients and their families on therapy exercises and the use of therapeutic devices, ensuring they are equipped to continue their rehabilitation outside of therapy sessions.
• Manage caseloads efficiently, prioritizing patient needs and coordinating daily staffing and assignment adjustments as necessary.

Patient Population / Client Focus

You will work with a diverse range of patients, tailored to both pediatric and adult populations, addressing orthopedic conditions, post-operative recovery, chronic pain, neurological disorders, and more. Each appointment offers the opportunity to work one-on-one with patients, fostering trust and encouraging open communication to maximize their rehabilitation potential.

Qualifications

• Must hold a valid Physical Therapy license.
• Completion of a Doctor of Physical Therapy (DPT) degree.
• Basic Cardiac Life Support (BCLS) certification must be obtained within 30 days of the employment start date.

Preferred Experience

• A minimum of two years of relevant physical therapy experience is preferred, including experience with home care delivery.
• Proven track record in developing and implementing effective individualized care plans.

Compensation and Benefits
