Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify emotional, social, and environmental factors impacting clients’ well-being.

• Develop and implement individualized care plans that align with clients’ needs, preferences, and goals.

• Offer compassionate counseling and emotional support to individuals and families navigating complex life challenges, including end-of-life issues.

• Collaborate closely with clients to set realistic goals, create actionable plans, and connect them with essential community resources.

• Provide crisis intervention and safety planning when necessary, ensuring clients’ immediate needs are addressed.

• Work with a multidisciplinary team, attending case conferences and team meetings to ensure holistic and integrated care delivery.

• Engage in community outreach initiatives to increase awareness of our services and the needs of the populations we serve.

• Participate in continuous professional development activities, including training and workshops, to enhance service delivery and clinical skills.

Patient Population / Client Focus\n

In this role, you will focus primarily on clients facing terminal illnesses, grief, and transition challenges. You will assist individuals and families through profound emotional experiences, providing empathetic listening, guidance, and access to essential support services. Your commitment to trauma-informed care principles will ensure a respectful and informed approach to each client’s unique circumstances.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in the state of Oregon.

• Strong foundation in social work principles, ethics, and practices, particularly in the hospice and palliative care settings.

• Demonstrated knowledge of community resources, socio-economic justice issues, and best practices in trauma-informed care.

• Exceptional interpersonal skills with an emphasis on communication, empathy, and cultural competence.

Preferred Experience\n

• Previous experience in a social service agency, hospice, or healthcare setting is favored.

Compensation and Benefits\n

This position offers a competitive salary ranging from $60,000 to $100,000 annually, commensurate with experience, along with a sign-on bonus to welcome you to our team.
