Key Responsibilities\n

As a Licensed Clinical Social Worker at Thrive Collective Health Center, your day-to-day responsibilities will include:

• Conducting comprehensive intake assessments while utilizing standardized assessment tools to determine the mental health needs of clients receiving hospice care.

• Providing individual counseling sessions through various platforms, ensuring each patient receives personalized support tailored to their unique circumstances.

• Collaborating closely with healthcare professionals, including nurses, physicians, and chaplains, to create and implement biopsychosocial treatment plans that address multifaceted client needs.

• Facilitating family therapy sessions, helping family members cope with the emotional and psychological stresses related to end-of-life care.

• Actively advocating for clients, assisting them in developing and achieving their personal goals related to mental health and overall well-being.

• Maintaining accurate and timely documentation in compliance with organizational policies and state regulations, ensuring seamless communication within the healthcare team.

• Engaging in continuous professional development activities to enhance clinical skills and maintain licensure.

Patient Population / Client Focus\n

Our primary focus is individuals and families dealing with terminal illness and hospice care. Qualifications\n

• A Bachelor’s degree in Social Work from an accredited institution is required, though a Master’s degree in Social Work is highly preferred.

• Current Licensed Clinical Social Worker (LCSW) designation in Oregon, with an unblemished disciplinary record.

• Solid clinical skills, particularly in end-of-life issues, assessment, diagnosis, and intervention planning.

Compensation and Benefits\n

At Thrive Collective Health Center, we recognize the value of your expertise and the vital role you play in supporting our clients. We offer a competitive salary range of USD 115,000 – 130,000 yearly based on your experience and qualifications. Additionally, our benefits package includes:
