Key Responsibilities\n

As a Licensed Clinical Social Worker, your responsibilities will include:

• Conducting thorough assessments of clients' counseling needs, particularly focusing on those affected by substance abuse and requiring mental health interventions.

• Utilizing DSM (Diagnostic and Statistical Manual) criteria to evaluate diagnoses and ASAM (American Society of Addiction Medicine) criteria to determine the appropriate care level.

• Creating customized treatment plans that outline the substance abuse counseling components necessary for client healing and recovery.

• Developing comprehensive discharge and continuum of care plans that facilitate clients' movement to suitable levels of care while preserving their dignity and self-respect.

• Meticulously documenting all counseling sessions, assessments, referrals, and follow-ups in the client's clinical record to ensure transparency and continuity of care.

• Managing a personal caseload that includes regular updates for case management and utilization reviews, forming a vital part of the care coordination process.

• Coordinating additional support services for clients and their families, enhancing the treatment experience.

• Establishing communication with referral sources such as legal, educational, or employment entities to create a comprehensive support network for clients.
