Work Environment\n

This position is fully remote, allowing for flexibility in your work schedule while maintaining a structure conducive to focused therapeutic engagements. You will work Monday to Friday from 9:00 AM to 5:00 PM, contributing to a dynamic team that values communication, continuous improvement, and the well-being of both its patients and staff.

Key Responsibilities\n

• Conduct comprehensive, age- and culturally-appropriate bio-psychosocial assessments to evaluate the mental health, substance use, and medical needs of patients in the hospice setting.

• Provide direct behavioral health services, inclusive of counseling, intake evaluations, crisis intervention, and treatment planning tailored to individual, couples, and family dynamics.
\n
• Document all interactions and assessments electronically, ensuring compliance with relevant state regulations and organizational protocols.
\n
• Facilitate referrals for community resources, including medication assistance programs, to ensure that patients receive comprehensive support tailored to their unique needs.

• Engage in collaborative efforts with Quality Improvement teams to incorporate data collection processes that enhance service delivery and improve patient outcomes.

Patient Population / Client Focus\n

As part of our hospice care team, you will primarily work with patients facing terminal illness and their families, addressing complex bio-psychosocial needs while providing emotional support. This population often requires nuanced understanding and sensitivity, as their end-of-life journey can be challenging. The skills you bring will help facilitate discussions concerning advance directives, grief counseling, and emotional support, ensuring that both patients and families feel valued and understood during this critical time.

Qualifications\n

• Valid and active Licensed Clinical Social Worker (LCSW) designation in the state of California.

• A Master's degree in Social Work from an accredited institution, combined with a minimum of three years of relevant post-licensure experience.

Compensation and Benefits\n
