Key Responsibilities\n

• Conduct in-depth mental health assessments through psychosocial evaluations, diagnostic interviews, and risk assessments to identify patient needs and treatment goals.

• Work collaboratively with patients to develop tailored treatment plans that encompass their individual circumstances, cultural backgrounds, and therapeutic needs.

• Utilize evidence-based therapeutic modalities such as cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and solution-focused brief therapy (SFBT) in various settings, including individual, couple, family, and group therapy.

• Provide timely crisis intervention services for patients experiencing acute distress, coordinating with healthcare professionals to assure continuity of care.

• Maintain meticulous clinical documentation, ensuring compliance with clinic policies and regulatory standards including progress notes, treatment plans, and discharge summaries.

• Offer psychoeducation to patients and their families, equipping them with strategies to effectively manage mental health challenges.

• Engage in regular team meetings to collaborate with other healthcare professionals, such as case managers and nurses, to coordinate comprehensive patient care.

• Conduct routine outcome assessments to measure the effectiveness of the proposed treatments and adjust plans as necessary to optimize patient outcomes.

• Stay current with developments in the field of mental health by incorporating best practices and emerging research into clinical strategies.

• Uphold ethical standards and ensure patient confidentiality is maintained consistently in all interactions.

Patient Population / Client Focus\n

You will work primarily with patients in hospice care, who often face significant emotional and psychological challenges due to chronic illness and the end-of-life process. Qualifications\n

• A Master's degree in Social Work is required.

• An active and valid State license to practice as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
