This position operates in a remote setting, allowing you the flexibility to work from home while being an integral part of our hospice care team. Key Responsibilities\n

• Conduct clinical assessments and evaluations to identify patient needs, establishing a foundation for tailored care plans.

• Facilitate discharge planning processes from the moment of patient admission, ensuring everyone involved understands the transition plan, resources available, and coping strategies for the patient and family.

• Lead psychoeducational and therapy groups, tailoring each session to address specific challenges faced by patients and their families.

• Actively collaborate with treatment teams to manage crises effectively, using evidence-based interventions to stabilize situations as they arise.

• Ensure continuity of care by maintaining communication with outpatient providers and referral sources throughout the patient care journey.

• Provide mentorship and support to milieu staff, enhancing their interactions with patients to promote a nurturing environment.

• Document all clinical notes, treatment plans, and assessments with proficiency in electronic health record systems, adhering to compliance and regulatory requirements.

• Participate in program outreach efforts, enhancing community awareness of hospice services and our organization's mission.

• Adapt to the dynamic needs of patients and families while proactively participating in care team discussions to improve treatment outcomes.

Patient Population / Client Focus\n

In this role, you will primarily focus on patients receiving hospice care, who often face complex emotional and psychological challenges related to their illnesses. Your expertise will help navigate these sensitive issues, support family members, and advocate for the patient's wishes and quality of life.

Qualifications\n

• A Master’s Degree in Social Work, Counseling, Psychology, or a related mental health discipline.

• Active and independently licensed as a Licensed Clinical Social Worker (LCSW) in California.

• Strong ability to set and maintain appropriate boundaries while establishing rapport with clients.

• Proficiency in utilizing Office 365 and Electronic Health Record (EHR) systems for documentation and communication.

• Exceptional written and verbal communication skills to convey complex information clearly and effectively.
