Key Responsibilities\n

As a Licensed Clinical Social Worker at Bedford Healthcare Center, your primary responsibilities will include:

• Conducting detailed and culturally sensitive bio-psychosocial assessments that consider the mental health, substance use, and medical needs of patients in a hospice setting.

• Providing individual, couples, and family counseling services, facilitating intake processes, performing treatment planning, and conducting crisis interventions as necessary.

• Performing regular mental health evaluations, documenting progress notes, and maintaining accurate records for all patient interactions.

• Acting as a liaison to connect patients with community resources, including assistance programs for medication, housing, and support services.

• Collaborating with Quality Improvement teams to gather and analyze data that contributes to the continuous improvement of care processes and patient outcomes.

Patient Population / Client Focus\n

You will primarily work with patients in hospice care, who are often facing significant life-limiting illnesses. This role demands not only clinical expertise but also empathy and understanding to navigate the complex emotional landscape patients and their families encounter. Qualifications\n

The ideal candidate for this position will possess:

• A current and valid LCSW license in the state of California.

Compensation and Benefits\n
