Key Responsibilities\n

• Collaborate with healthcare team members to develop comprehensive treatment plans, including the formulation of discharge plans based on detailed psychosocial assessments and evidence-based interventions.

• Engage and build strong relationships with community resource leaders by representing Redesign Counseling Hub at meetings and on committees that focus on addressing the social needs of our patients.

• Provide individual counseling and crisis intervention for patients, conducting rigorous assessments to address and treat emotional and behavioral challenges that arise.

• Effectively communicate with physicians regarding patient progress, needs, and concerns, and participate actively in treatment plan discussions to ensure coordinated care.

• Maintain accurate clinical records and documentation, including progress notes, in compliance with department policies to support quality improvement initiatives.

Patient Population / Client Focus\n

This role will involve interacting with a diverse patient population within the Hospice Center, assisting individuals facing end-of-life issues and their families. Qualifications\n

• A Master's degree in social work (MSW) and current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Oregon.
