Key Responsibilities\n

As an LCSW, your daily responsibilities will include:

• Conducting comprehensive psychosocial assessments to identify the individual needs and emotional challenges of hospice patients and their families.

• Developing personalized care plans in collaboration with clients, medical staff, and family members, aimed at addressing psychological, emotional, and social goals.

• Providing individual and family counseling to assist with coping strategies throughout the hospice journey, including grief counseling, crisis intervention, and conflict resolution.

• Collaborating with an interdisciplinary team of healthcare professionals, including nurses and physicians, to coordinate and optimize holistic care.

• Facilitating access to community resources, support groups, and services, ensuring patients and families are informed of available benefits.

• Monitoring client progress and making necessary adjustments to care plans based on ongoing assessments of their mental and emotional states.

• Documenting client interactions, assessments, and care plans accurately within our electronic health record (EHR) system in compliance with state regulations.

• Providing on-call support for urgent situations, offering immediate assistance to families when needed, even outside regular work hours.

Your primary focus will be on hospice clients, particularly seniors facing terminal illnesses. You will work with families navigating complex emotions and decisions in a challenging time, delivering individualized support that respects their unique cultural and personal backgrounds. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Active and valid state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
