Description: The Adult Nurse Case Manager coordinates care for a select caseload of patients. This involves triage, care coordination, and ensuring safe transfer to home. Proactive communication with payors is essential to guarantee approval of treatment plans and required resources. Knowledge of clinical systems and fiscal data related to specific patient populations is maintained. Collaboration with the multi-disciplinary team and patients/families is crucial for effective transition planning. The role includes coordinating transitions to post-acute settings such as nursing facilities and inpatient rehab. Patients are managed in accordance with departmental procedures, ensuring clinical safety and quality. Patient information is entered and maintained in all departmental systems, adhering to operational guidelines. It is important for Bedside Case Managers to have a current/active CPR card. Med Test is not required as there is no access to or administration of medications to patients.
