Physical Therapist (Home Health PT) Full-Time TX - Austin Home Health Branch, Austin, TX, US 14 days ago Requisition ID: 5231 Choice Health at Home is seeking a compassionate and skilled Physical Therapist to provide in-home rehabilitative care. The Physical Therapist evaluates, develops, and implements individualized treatment plans to improve mobility, reduce pain, restore function, and enhance patients' quality of life. Responsibilities Complete initial patient evaluations and develop individualized treatment plans. Provide physical therapy services in the patient's home. Monitor patient progress and modify treatment plans as needed. Educate patients and caregivers on exercises, safety, and disease management. Maintain timely and accurate clinical documentation. Collaborate with physicians and interdisciplinary team members to ensure quality patient care. Qualifications Graduate of an accredited Physical Therapy program. Current Physical Therapist license. CPR certification required. Home health experience preferred. Homecare HomeBase experience preferred. Valid driver's license, reliable transportation, and auto insurance.
