Clinical Manager - Home Health

Job Summary Manages clinical staff and oversees clinical activities for assigned department, including patient care, record‑keeping, budgeting, and personnel issues. Assigns patients to available staff based on patient condition and staff skill level, schedules staff shifts, measures performance, manages workflow volumes, and applies continuous improvement processes to enhance quality. Key Responsibilities Schedule clinical staff for specific shifts, considering past utilization trends and patient needs. Measure employee performance and assist in training new staff and developing existing staff members. Manage workflow volumes, monitor work queues, and ensure timely processing. Apply continuous improvement processes and evaluate their impact on quality. Monitor patient care areas and facilities for cleanliness, readiness, and compliance. Provide care to patients on an as‑needed basis and support special projects as assigned by leadership. Direct clinical staff activities for patients receiving home health and/or hospice services in the assigned service area. Ensure effectiveness and efficiency of home health and hospice services, delegating responsibilities to clinical and professional staff. Provide direction on clinical practice and regulations, input for planning and budgeting, and coordinate interdisciplinary communication among team members. Act as an internal liaison to Sanford and external agencies, collaborating with the communities served. Qualifications Appropriate education level in accordance with state licensure requirements. Minimum two years of therapy experience caring for patients, residents, or clients. Minimum two years of management experience preferred. Licensed in the state(s) of practice as a Physical Therapist or Occupational Therapist. Basic Life Support (BLS) certification may be required for frequent patient contact and patient care.
