Clinical Manager - Home Health

Role Description The Clinical Manager - Home Health is a part‑time (with the option of converting to full time), on‑site role based in Lowell, MA. This role oversees daily clinical operations for home health services, including coordinating patient admissions, managing caseloads, and ensuring timely, high‑quality care in accordance with regulatory and organizational standards. The Clinical Manager provides clinical supervision and support to nursing and interdisciplinary team members, conducts case conferences, and reviews documentation for accuracy and completeness. Responsibilities include implementing quality improvement initiatives, monitoring clinical outcomes, and addressing service issues or concerns raised by patients, families, or team members. The role also involves onboarding, training, and mentoring clinical staff, collaborating with physicians and referral sources, and participating in strategic planning to enhance home care services. Qualifications Education Licensure and Education: Current RN license in good standing (or equivalent clinical licensure as required), with a bachelor’s degree in Nursing or a related field preferred. Experience Clinical Operations: Demonstrated experience managing clinical workflows, staff scheduling, and caseload coordination within a home health or related care setting. Home Care and Medicine: Strong knowledge of home health regulations, clinical best practices, and evidence‑based care for acute and chronic conditions in a home environment. Training and Leadership: Proven ability to train, mentor, and support clinical staff, provide performance feedback, and foster a culture of accountability and collaboration. Quality Improvement: Experience designing, implementing, and monitoring quality improvement initiatives, including audits, outcome tracking, and process optimization. Communication and Coordination: Excellent interpersonal, documentation, and care coordination skills, with the ability to work effectively with patients, families, and interdisciplinary teams. Regulatory and Compliance: Familiarity with home health accreditation standards, state and federal regulations, and payer requirements. Other Prior supervisory experience in home health or community‑based care strongly preferred. Valid driver’s license and ability to travel locally for home visits as needed. #J-18808-Ljbffr
