Registered Nurse / Patient Care Manager, Home Health

We are seeking a Patient Care Manager responsible for overseeing patient care quality and ensuring compliance within a healthcare setting. Key Duties Include Providing clinical supervision to ensure care aligns with standards, policies, and regulations Coordinating and monitoring patient care for continuity and quality Supervising clinical staff to ensure consistent service delivery Maintaining effective communication with patients, caregivers, referral sources, and staff Driving quality outcomes through departmental goals and quality improvement initiatives Modeling professionalism, service excellence, and organizational values Ensuring compliance by completing training, accurate reporting, and cooperating with audits Promoting a safe work environment by following safety protocols Supporting staff development via coaching, mentorship, and ongoing education Qualifications include Current registered nurse licensure in the practicing state Experience in home care with at least two years of management or supervisory roles preferred Knowledge of professional standards, Medicare Conditions of Participation, and regulatory requirements #J-18808-Ljbffr Preferred Home Health Care & Nursing Services
