Work Environment\n

This role is predominantly remote, allowing for flexibility while maintaining strong engagement with your team and patients. Despite the remote setting, our organization promotes a collaborative atmosphere where mental health professionals work closely with medical staff to coordinate patient care effectively. The work schedule is Monday to Friday from 8:00 AM to 5:00 PM, with occasional evening shifts required to meet patient needs.

Key Responsibilities\n

• Conduct comprehensive mental health assessments to evaluate the psychological impacts of patients' chronic medical conditions.

• Develop and implement individualized treatment plans that align with both psychiatric and medical objectives.

• Facilitate individual and group therapy sessions using evidence-based therapeutic modalities tailored to the specific needs of each patient.

• Maintain accurate and compliant patient documentation following legal, ethical, and organizational standards.

Patient Population / Client Focus\n

Your work will include serving a diverse patient population, primarily those dealing with co-occurring mental health disorders and chronic medical issues. You will engage with individuals navigating complex life challenges, requiring a deep understanding of how these interrelative factors affect their overall well-being. This role emphasizes a patient-centered approach, ensuring that care is tailored to the unique contexts of each individual.

Qualifications\n

• A Master’s or Doctoral Degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Current and valid licensure as an LCSW, LCPC, or LMFT in the state of Illinois.

• Proficient in addressing the mental health needs of individuals with complex, co-occurring medical conditions.

• Demonstrated experience in providing integrated care within a multidisciplinary team setting.
\n
• Excellent interpersonal and communication skills, able to connect with patients and collaborate effectively with healthcare providers.

• Commitment to maintaining ethical guidelines and patient confidentiality, with experience using electronic health record systems.

• Strong teamwork abilities and a proactive approach to problem-solving and patient care.

Preferred Experience\n

• At least two years of relevant experience in a mental health counseling setting, particularly in outpatient or integrated care services.

• Experience in therapeutic modalities specific to chronic pain management, substance use, and other co-occurring disorders.

• Familiarity with evidence-based practices and tools commonly used in mental health therapy.

Compensation and Benefits\n
