Community Health Worker: Resource Navigator

BJC HealthCare in St. Louis is seeking a Community Health Worker to improve preventive health outcomes by partnering with an interdisciplinary care team and community resources. You will identify gaps in care and help clients access services, document plans, and follow up with patients in person or by phone.
