Key Responsibilities\n

• Conduct thorough psychosocial assessments through detailed interviews, observations, and medical record reviews, identifying emotional, social, and environmental factors impacting students' rehabilitation.

• Deliver compassionate counseling and support to patients and their families, helping them adjust to their new circumstances and addressing barriers to emotional well-being.

• Work closely with an interdisciplinary rehabilitation team to develop comprehensive, individualized treatment plans that integrate psychosocial insights and facilitate coordinated care.

• Assist patients in navigating community resources, advocating for access to essential services such as support groups, vocational rehabilitation programs, and housing assistance.

• Provide education and ongoing support to families regarding community resources available post-rehabilitation, ensuring a seamless transition and sustained growth.

• Participate actively in discharge planning, working collaboratively to facilitate smooth transitions to home or alternative care settings based on the patient’s needs.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Current, active state licensure as a Licensed Clinical Social Worker (LCSW) in Tennessee.

Compensation and Benefits\n
