Key Responsibilities\n

• Conduct thorough psychosocial assessments through patient interviews, observational data, and review of medical records to determine the emotional, social, and environmental factors affecting recovery.

• Provide empathetic counseling and support to patients and their families, focusing on issues related to emotional adjustment and resilience during transitions.

• Actively collaborate with rehabilitation and medical teams to design and implement individualized treatment plans that address patients' psychosocial needs.

• Guide patients in accessing relevant community resources, including support groups and vocational rehabilitation, to enhance their recovery journey.

• Educate families about available community resources and post-rehabilitation support, facilitating smooth transitions back into everyday living.

• Participate in discharge planning processes, assisting in the coordination of care for continuity post-treatment.

Patient Population / Client Focus\n

Our clinic serves a diverse group of patients, many of whom face challenges stemming from strokes, spinal cord injuries, orthopedic surgeries, and various neurological disorders. You will engage with patients from varied backgrounds and age groups, providing tailored support to promote their emotional health and successful rehabilitation.

Qualifications\n

• Master's degree in Social Work from an accredited institution is required.

• Currently possess an active state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n

The annual salary for this position ranges from USD 75,000 to 100,000, complemented by a sign-on bonus to welcome you to our team.
