Transition Coordinator
DGS Office of External Affairs
The Division of Developmental Disabilities seeks a Social Worker IV to serve as Transition Coordinator at Murray Developmental Center in Centralia, Illinois. This position involves developing and coordinating Individual Service Plans, conducting social evaluations, managing admissions and discharge planning, and serving as a lead worker for interdisciplinary teams. The coordinator will provide consultations to community agencies and perform casework related to program compliance and transition services for individuals with intellectual disabilities. Key qualifications: Master's degree in social work from a recognized school of social work Three (3) years of supervised post-master's clinical social work practice Clinical social worker's license from the Department of Professional Regulation Ability to work after business hours, weekends, and holidays Ability to pass IDHS background check and drug screen Ability to travel with overnight stays as appropriate #J-18808-Ljbffr DGS Office of External Affairs
