Through our talented staff and “LEAP” career framework (Leadership, Empowerment, Accountability, Personal Growth), we are committed to a people-first approach that fosters trust, inclusion, growth, and development for all.The Chinatown YMCA is seeking a Case planner who will be responsible for providing direct counseling for multi-problem minority families.Key Responsibilities:Provide direct preventive services including individual, group, and family counseling to assigned caseload of a minimum of 12+ families. Evaluate psychosocial needs through regular home visits and other appointments based in the child/ren needs in a school setting.Conduct intake assessment of cases assigned by supervisor in order to obtain, analyze, and evaluate information related to client’s psychosocial problems and functioning.Provide crisis intervention as necessary when client/family is in an emergency at the school. Secure concrete support services to each child.Maintain accurate and up-to-date uniform case record, progress notes, and other records for all clients in accordance with deliver high quality youth and family counseling program under the guidelines of the SBES/ACS (Administration for Children Services) and/or specific funding source.Create a positive relationship with ACS staff, OCFS staff, school guidance counselors, teachers, and fellow social work agencies.Provide positive and engaging casework practice that meets the ACS, NYS, and OCFS standards resulting in positive program outcomes, and positive audits/reviews and complying with the guidelines of the program and YMCA.Specific casework practice will result in increased, engaged school children and families; engage families, including self-referrals, to discuss program offerings and expectations.Collaborate with referral sources to understand each family's unique strengths, needs, and priorities, ensuring a good program fit; secure informed consent from families for information sharing to facilitate service delivery.Conduct comprehensive assessments to identify families' strengths, needs, and service requirements; partner with families to develop individualized support plans based on assessment findings.Connect families with a full range of services, including those offered by the program, schools, and community organizations (mental health services, essential needs support, child development programs, pre-and postnatal services, support groups, and resource networks).Advocate for families as they navigate complex systems and access government and private-sector services; facilitate open communication and collaboration among service providers involved with the families; provide ongoing support to families transitioning out of the program.Advocate for families' involvement in their children's education; assist families with meeting basic needs (income, food, housing, transportation, and other concrete needs); guide families in accessing essential community services and public benefits.Handle a caseload of 12+ and be able to maintain documentation updates, progress notes, and 30, 90, and 6-months Assessments; be able to meet for supervision every week to assess families and community needs.Desired Skills & Experience:Bachelor's degree in Counseling, Psychology, or Sociology.One (1) to two (2) years of related experience.School-Based Early Support - Catchment 1, 2, 3 (Districts 1 & 2).
