We are currently looking to hire a passionate and service-minded leader to serve as our Assisted Living Care Coordinator and become a key member of our health services team! Our ideal applicant will have a CNA license and have weekend availability.

Responsibilities for our Resident Care Coordinator include the following:

• Filling other gaps in CNA/Med Tech schedule, as needed fill in for regular shifts.
• Work side by side assisting the Director of Nursing.
• Conference with families as needed.
• Maintaining MAR, handling communication regarding medications, cycling medications, and maintaining an organized medication cart.
• Coordinating with Home Health and Hospice Companies and other providers.
• Assisting with CNA/Med Tech In-service training.
• Follow up on shift change notes.
• Communicating regularly with Nurse and Executive Director.
• Fulfilling other duties as assigned by administration.
• Ordering supplies as needed.
• Organizing vaccine clinic and yearly immunization form.
• Manage budget and spenddown.

Qualifications

Qualifications:

• Must have current CNA license
• Previous Med Tech experience
• Experience caring for elderly residents in an assisted living or nursing home setting.
• Great computer skills
• Problem solver
