Responsibilities
- Assess patient status and contribute to developing and implementing individualized PT treatment plans for residents in an LTAC/SNF setting.
- Deliver skilled PTA interventions including therapeutic exercises, gait training, range of motion, balance activities, transfers, and mobility training.
- Apply modalities and manual techniques as appropriate under supervision, monitoring patient response and safety.
- Document patient progress, update treatment plans, and communicate with the supervising PT and rehab team.
- Collaborate with nursing and therapy staff to support discharge planning and continuity of care for medically complex patients.
- Maintain adherence to facility policies, infection control practices, and safety standards in all patient care activities.

Requirements
- Active Physical Therapy Assistant license in Maryland or eligible for licensure in Maryland.
- Associate degree from an accredited PTA program.
- Experience in inpatient rehabilitation or skilled nursing facility/LTAC settings preferred.
- Ability to work in a LTAC/SNF environment and adhere to the stated schedule (8:00 a.m.-4:30 p.m., five days per week).
- Strong documentation and communication skills; ability to collaborate with a multidisciplinary team.
