Responsibilities
- Carry out prescribed physical therapy procedures and exercise programs under the supervision of a licensed physical therapist.
- Facilitate therapeutic activities focused on functional mobility, including seating, transfers, gait training, balance and strength work.
- Support data collection, monitor patient response to treatment, and document progress in accordance with facility policies.
- Assist with patient education and caregiver guidance to promote continued therapy outside formal sessions.
- Maintain a safe, organized therapy space and ensure equipment is used correctly and cleaned between patients.
- Communicate effectively with the physical therapy team, nursing staff, and other disciplines to coordinate care and support discharge planning.

Requirements
- Current Physical Therapy Assistant credential or eligibility to practice
- Experience in LTAC or skilled nursing facility settings is preferred
- Ability to work within a multidisciplinary team and follow a plan of care under supervision
- Strong documentation and communication skills
- Availability for the stated shift and travel assignment
