Work Environment

This role is primarily onsite, requiring visits to clients' homes across Ithaca and surrounding areas. Key Responsibilities

• Conduct thorough assessments of residents, evaluating their physical capabilities and identifying specific treatment needs.
• Craft and execute customized treatment strategies aimed at improving strength, mobility, and overall independence.
• Deliver a variety of therapeutic interventions, including exercises tailored to individual capabilities and manual therapy techniques that promote recovery.
• Keep detailed records of therapy sessions, documenting patients' progress, and adjusting treatment plans as warranted.
• Collaborate closely with an interdisciplinary team of healthcare professionals, including doctors and occupational therapists, to maximize the efficacy of care.
• Provide educational resources to clients and their families, helping them integrate home exercise regimens into their daily lives.
• Adhere to all state and federal guidelines, ensuring the highest levels of compliance and professional standards in your practice.

Patient Population / Client Focus

In this role, you will primarily work with homebound clients, many of whom are elderly and may have complex rehabilitation needs. This patient population requires not only clinical expertise but also empathy and strong communication skills to foster trust and encourage adherence to treatment plans. You'll be instrumental in helping these residents regain function, reduce pain, and improve their overall physical health.

Qualifications

• Valid state licensure as a Physical Therapist is required.
• Bachelor's Degree in Physical Therapy from an accredited institution; Master's or Doctorate preferred.
• Demonstrated ability to work under varying degrees of supervision and exercise professional judgment.

Preferred Experience

• A minimum of two years of experience in a clinical setting, preferably within home health or skilled nursing facilities.
• Experience working with geriatric patients is highly valued.
