Work Environment\n

This position offers flexibility with a remote work setting, allowing for a healthy work-life balance while serving clients who require specialized support. Although primarily remote, you may occasionally engage in community visits to perform assessments and provide hands-on assistance. Operating on a standard full-time schedule of Monday to Friday from 8:00 AM to 4:30 PM, we understand that some client needs may require adaptations, including occasional evening or weekend hours. Flexibility is essential, as on-call availability may sometimes be necessary for addressing urgent situations.

Key Responsibilities\n

As a Licensed Clinical Social Worker, you will be pivotal in assessing and addressing the psychosocial needs of our senior clientele. Your responsibilities will encompass:

• Collaborating closely with clients, their families, and care teams to develop and implement tailored care plans.

• Maintaining accurate documentation of assessments, care plans, and progress notes, utilizing electronic health record (EHR) systems efficiently.

Patient Population / Client Focus\n

Your primary focus will be on seniors within our community who face various challenges, including mental health disparities, chronic illness, and social isolation. You will serve clients from diverse backgrounds, requiring cultural competence and sensitivity in your approach. Understanding family dynamics and facilitating effective communication among family members will also be an integral part of your role.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A current and valid state license as a Licensed Clinical Social Worker (LCSW).

• A minimum of three years of experience in a clinical social work role, with preference given to those experienced in geriatric or community health settings.

Preferred Experience\n
