Key Responsibilities\n

• Conduct thorough biopsychosocial assessments to evaluate mental health, substance use, and medical concerns of patients in the hospice setting.

• Develop and implement individualized treatment plans, including crisis intervention, intake assessments, and therapy (individual, family, and group sessions), tailored to address specific patient needs.

• Record and maintain accurate clinical documentation within an electronic health record (EHR) system, ensuring compliance with organizational standards and regulations.

• Collaborate with healthcare teams to ensure continuity of care and escalate patient needs to appropriate resources as required.

• Assist in the coordination of referrals to community resources, including mental health services, support groups, and medication assistance programs.

• Engage in activities focused on quality improvement, such as developing protocols for data collection and assessing service effectiveness.

Patient Population / Client Focus\n

As an LCSW with Uplift Sobriety Counseling Solutions, you will work predominantly with individuals facing terminal illnesses in a hospice setting. Qualifications\n

• A Master’s degree in Social Work, Counseling, or a closely related field is required.

• A valid Licensed Clinical Social Worker (LCSW) license issued by the state of California.

Compensation and Benefits\n
