Requirements: 1 year home health experience; SSN required; DOB required; References: 1 reference in entire work history; Driver's License and Auto Insurance

Responsibilities:

• Provide outpatient physical therapy services to patients in a hospital setting.
• Assess patient needs and develop individualized treatment plans.
• Document patient progress and communicate with healthcare team.
• Maintain compliance with internal policies and regulations.
• Participate in weekend and holiday coverage as scheduled.
