Key Responsibilities\n

• Conduct thorough psychosocial assessments utilizing interviews, observations, and medical records to evaluate emotional, social, and environmental factors that impact patients’ recovery and rehabilitation.

• Provide individualized counseling and emotional support to patients and their families, particularly in dealing with adjustment issues related to illness and grief.

• Collaborate closely with rehabilitation professionals, including physical and occupational therapists, to formulate tailored treatment plans that address patients’ unique psychosocial needs.

• Assist patients in navigating community resources, such as support groups, vocational rehabilitation programs, and housing assistance, ensuring they are well-equipped for post-rehabilitation services.

• Educate and empower patients and their families regarding available community resources, fostering a supportive network for their healing journey.

• Play an essential role in discharge planning and facilitate smooth transitions to home or other care settings, ensuring that patients continue to receive the necessary support.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

Compensation and Benefits\n
