Overview In-Office/Onsite Position Advance Practice Provider (Physician Assistant / Nurse Practitioner Masters / Clinical Nurse Specialist Masters) Employment Type : Full Time Benefits : M/D/V, Life Ins., 401(k), PTO, Paid Holidays, Bonus Programs SCOPE: Under supervision or in collaboration with practice physicians, the Advance Practice Provider involved in the care of individuals, families and patient groups who are dealing with cancer or other related diagnosis. This care may be in clinic or virtually, via telemedicine. Responsibilities may include duties related to prevention, screening, patient care, management, and research. May serve as direct caregiver, health care consultant, patient educator, administrator, or researcher. Ensures compliance with OSHA and other regulatory standards and requirements. Supports and adheres to the US Oncology Compliance Program, to include the Code of Ethics and Business Standards. Northwest Cancer Centers is the most advanced oncology group in Northwest Indiana. Our Gynecologic Oncology team offers an integrative approach to the diagnosis, treatment and surgical management of women cancers. We believe that long-term care after cancer treatment is as important as your current treatment. Responsibilities ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Assesses, plans, implements,and evaluates care for patients.
• Initiates and documents appropriate interventions; evaluates and records patient/family responseandprescribes therapiesas indicated.
• Performs proceduresafter appropriate training.
• Assesses patient/family needs and provides education.
• Promotes wellness and recommends community resources to meet patient and family needs.
• Incorporatesthedevelopingknowledge ofevidence-based research into clinical practice.
• Developingknowledge of reimbursement procedures and documentation requirements.
• Completes patient chargecaptureandmedical record documentationin a timely fashion.
• May provide case management forpatientsand their families, given limited knowledge of community resources.
• May promote/provide staff education.
• Responsibilities may include rounding at affiliated hospitalsand weekend or after-hour calls.
• Other duties as assigned.

Qualifications MINIMUM QUALIFICATIONS: Nurse Practitioner Masters of Science in Nursing (MSN) from an accredited nursing school and successful completion of a Nurse Practitioner program.

• This position is entry-level and typically requires a minimum of 0-3 years post MSN experience, preferably in the chosen specialty.
• RN state licensure and current registration with the State Board of Nursing in practicing state.
• Current state licensure as a Nurse Practitioner.
• Current basic life support (BLS)/CPR certification required.

OR Physician Assistant with successful completion of an accredited Physician Assistant program.

• This position is entry-level and typically requires 0-3 years of experience as a practicing Physician Assistant, preferably in the chosen specialty.
• Current NCCPA (National Commission on Certification of Physician Assistants) certification.
• Current state license as a Physician Assistant in practicing state. Current basic life support (BLS)/CPR certification required.

OR Clinical Nurse Specialist Masters of Science in Nursing (MSN) from an accredited nursing school and successful completion of a Clinical Nurse Specialist program.

• This position is entry-level and typically requires a minimum of 0-3 years post-MSN experience, preferably in the chosen specialty.
• RN state licensure and current registration with the State Board of Nursing in practicing state.
• Current state licensure as a Clinical Nurse Specialist. Current basic life support (BLS)/CPR certification required.
