What You’ll Do As an RN Case Manager , you'll be the heart of the care team, serving as a trusted advocate for patients and families while coordinating personalized hospice services. In this role, you will: Assess patient and family needs and develop individualized care plans Deliver skilled nursing care focused on comfort, symptom management, and quality of life Coordinate care among physicians, hospice aides, social workers, chaplains, and other team members Educate and support families through every stage of the hospice journey Supervise and guide hospice aides to ensure exceptional patient care Maintain accurate clinical documentation and participate in ongoing professional development What You'll Bring We're looking for a nurse who combines strong clinical skills with a servant's heart. Qualifications: Current Virginia RN license (single-state or multistate) CPR certification Minimum two years of nursing experience Hospice, palliative care, or home health experience preferred Strong communication, assessment, and organizational skills Valid driver's license, reliable transportation, and automobile insurance A compassionate, self-motivated approach to patient-centered care What We Offer $10,000 Sign-On Bonus* Because great nurses deserve great rewards.
